To place an order:

1.  Click on this ORDER FORM link.

2.  Complete form online.

3.  Print off the order form. 

4.  If you would like to design your own layout, please print off this template follow the

    directions on the spreadsheet. 

5.  Collect your shirts for the quilt. 

6.  Make sure to include the order form, layout template (if needed) and any special   

     instructions.

7.  Include a check for the down payment. ($100 per quilt)

8.  Drop off the shirts or send them via UPS so you can track your order.
